You must have a NOTARIZED release on file at our front gate. The new forms may be downloaded from this website, under Application Forms. BOTH parents must sign it and have it notarized, unless ONE parent has legal guardianship. We are having all of our front gate people become Notary Publics, but it may take a little longer, so please download the form and have it taken care of.
